Background and Objectives: Chronic anxiety and depression are common in hemodialysis patients, impairing their quality of life and treatment adherence. This study aimed to evaluate the effectiveness of spirituality-based mindfulness training (S-MBT) in reducing these symptoms.

Methods: A randomized controlled trial was conducted. Thirty hemodialysis patients in Qom Province, Iran, in 2023 were recruited via convenience sampling and randomly assigned to either the intervention group (S-MBT; n=14) or the control group (n=16). The intervention group participated in an eight-week S-MBT program, consisting of weekly 90-minute sessions. Anxiety and depression levels were assessed using the Hospital Anxiety and Depression Scale at baseline and one month post-intervention. Data were analyzed using descriptive statistics and analysis of covariance.

Results: Participants in the intervention group showed significant reductions in anxiety (pre: 8.71±5.61; post: 5.92±3.28; P=0.023) and depression scores (pre: 9.62±3.34; post: 7.00±3.62; P=0.047) following the intervention. No significant baseline differences were observed between the two groups. Post-intervention, anxiety levels in the intervention group were significantly lower compared to the control group (P=0.001), with a marginally significant reduction in depression scores (P=0.088).

Conclusion: S-MBT appears to be a feasible and potentially effective intervention for alleviating anxiety and depression among hemodialysis patients. Further research with larger, multicenter randomized controlled trials is warranted to validate these findings and elucidate the underlying mechanisms.
